Healthcare Navigation

Our Healthcare Navigation, offers:
Care Coordination, Support and Communication, Education and Empowerment, Development and Organization along with Safety and Transitional Services.
Care Coordination
We provide coordination of care services to improve your quality of life. We do this by helping you schedule appointments, including referrals to specialists, managing prescriptions along with accessing appropriate home care or community services when needed.
Support and Communication
We can attend your medical appointments, procedures, and hospitalizations to help you communicate better with your provider(s), promote safety, and help reduce your anxiety, stress, and confusion. Prior to an appointment, we can help you prepare by making a list of your questions or concerns, to make sure they are addressed and that your voice is heard. We can take notes and provide you with a summary to help you understand what was discussed and to review the details as needed. We can also help communicate with your family to promote their understanding of your decisions, along with your healthcare providers.
Educate and Empower
We will promote your independence and empower you, by educating you about your health, to allow you to make informed decisions about your diagnosis and your treatment options including but not limited to procedures, treatment and medication therapy. We can also assist you in getting a second opinion if needed.
Develop and Organize
Our organization of your health, medical, and billing records, coordinated into one central healthcare summary, can help you be in control of your healthcare.  We can also assist you with understanding how to get your advance directives or living will completed.
Safety and Transitions
We help to reduce your risk from the thousands of medical errors that occur every day in the U.S. We accomplish this by bridging the gaps in your healthcare journey, we promote safer transitions between each care setting that you may encounter. We facilitate communication, make sure your voice is heard, and ensure that you receive the correct follow up care that you need.

Eldercare Services

Our Eldercare Services share our Healthcare Navigation Services: Care Coordination, Support and Communication, Education and Empowerment, Development and Organization along with Safety and Transitional Services. Seniors however, especially if alone, often require more diligence and care due to the aging process. Here we add more focus to Care Coordination, Safety and Communication
Care Coordination
Older adults experience more frequent care transitions than younger patients due to having multiple medical conditions and higher medication use. Transitions of care is the time that a person is at most risk of falling victim to a medical error. We use our knowledge, communication, resources and organization to assess your health information to reduce this risk.
We will research, educate, and inform you, so you can make the best-informed decisions for your treatment options, providers, and levels of care needs.
Safety and Communication
Overall safety and working through common transitions later in life, is often a concern for many older adults and their families. This can also lead to potential conflicts and misunderstandings between family members and providers.
We work closely with our clients and those family members they wish to involve, ensuring that their voice is heard. This could be related to their independence, quality of life, treatment options, aging in place, transitioning to assisted living or end of life decisions.
While working with older adults, we help reduce the pressure on their adult children who may be living too far away, busy  managing their own families and children, or busy with work and a hectic schedule. It can be challenging for most families to take the emotion out of helping their aging family members through difficult transitions. An Independent Patient Advocate can be the most helpful resource to effectively manage these times to improve health, safety and rights.

Medical Billing and Insurance

Our billing specialty is reviewing pharmacy charges and monthly statements for retail, mail order and VA consumers as well as for those living in long term care settings, who are receiving their medications administered by staff. After working in the pharmacy industry for 30 years, we have navigated thousands of cases to get consumers the best outcomes possible for the prescriptions that they needed. This is best done by including a medication reconciliation with billing reviews to ensure accuracy of day to day medications. This can also reduce waste and to help prevent medication errors.
Educate and Empower
We can review your certificate of coverage to help you fully understand what is covered and what is not covered by your insurance plan. From covered and non-covered services to prescriptions. We will describe to you the differences between the multiple out of pocket expenses related to your individual plan, from premiums to deductibles and co-insurance to co-pays.
If needed, we can review your plan and compare it to other options during the open enrollment time or during other special circumstance opportunities.

Review and Organization
Billing errors can be found in 80% of bills that patients receive, frequently overcharging healthcare consumers. We will help you organize your medical bills and review them to find potential errors. If errors are found, we can work to get them corrected and resolved.

We can help to negotiate payment plans, disputes and help to file an appeal if your insurance denies a claim or ends your coverage. Our expertise is knowing how to navigate the insurance system, who we need to talk to and how to get the answers we need. You can take comfort in knowing that once completed, your billing will be accurate and that you are paying the correct price to your provider.

Step 1:
Free 20 min consultation

The help that we provide is unique to you or your loved one’s needs.
To determine if our services are the best fit for you, we offer a free initial 20 minute phone call to briefly discuss your situation. Call 608-336-3312 today or e-mail steph@AVoiceForYouPA.com to get started.

Step 2:
Schedule a complete consultation

Given the opportunity, our next step is to schedule a time to perform a complete in-depth consultation. This typically lasts 1.5 to 2 hours. Prior to scheduling this meeting, we require payment of our consultation fee, and documentation to be reviewed, completed and signed electronically. This meeting can be scheduled to be in person, by phone or through video conferencing. Beforehand, we will provide you with a list of important information that we will need to gather from you before or during that meeting. We will be discussing in more depth, the details of your healthcare journey and refine your short term and long term goals. This will allow us to create your individualized advocacy action plan.
Step 3:
Receive your individualized plan

Following your completed consultation, you will receive a summary and a proposal for your advocacy action plan. Your action plan will include an estimated number of hours and length of time that we anticipate needing to reach your goals. Services may be short-term, long-term or as needed.  A retainer will be required, the amount due will be based on your estimate. Our services will be provided at an hourly rate, and an outline of services paid by your retainer will be provided monthly. We communicate regularly to keep you informed as time and services are provided to stay on track and to keep you informed. Together, we will continually monitor and reassess your goals so adjustments to the action plan can be made as needed.
